The test can be taken up to 5 times.

Once you have successfully passed the 'Clinical Mentoring MOOC Assessment' and the 'CPD Feedback Form', Please follow the 'Purchase Certificate' button below where you will be taken to the AWFDCP Bangor University shop.
Following verification, certificates will be issued to the email address associated with the purchase. This usually takes 1-3 working days.
Please ensure that the name and email address used to login to the assessments are the same as used to purchase the certificate
